How Do I Get My X-Rays?

Just ask nicely! You are legally entitled to copies of your dental X-rays and intraoral photos.

You probably already have a copy of the treatment plan the dentist is recommending (often called a “Financial Agreement”), but you can always ask for this as well.

You don’t have to explain why you are requesting the records.

Requesting records does not necessarily mean that you are leaving the practice, and you absolutely should feel comfortable returning to the office even after you request records.

To acquire copies of your records, you can send an email to the practice using this template:

Subject: Request for Dental X-rays

Body:

Dear [Dental Office Manager/Receptionist’s Name],

I am writing to request copies of the dental X-rays that were taken at my last appointment. Additionally, I would appreciate any intraoral photos taken as well as a summary of the proposed treatment plan.

If any specific procedures or forms are required to facilitate this request, please let me know, and I will promptly comply.

Thank you,
